Title : Hospice Admission RN
What Youll Do :
The Hospice Admission Nurse will be an expert in the admission process and be well versed in hospice eligibility. They will assist with hospice talks, providing information about the hospice benefit for potential hospice patients and their families.
- Provide skilled nursing services to the patient in their own home under the direction of a physicians plan of care.
- Develop plans of care with realistic / measurable outcomes in collaboration with the patient / family.
- Provide education and promote patients rights and responsibilities for informed decision making in the plan of care.
- Assess and identify the patients holistic health care needs.
- Demonstrate competency in assessment and technical skills.
- Possess and utilize knowledge of pain control & symptom management.
