We are seeking a highly motivated and compassionate Support Professional to join our team! As a Support Professional, you will work one on one with youth who have cognitive impairments and functional limitations, to build daily living and behavior management skills in both their homes and community settings.
Responsibilities:
- Develop and implement personalized treatment plans to promote healthy, developmentally appropriate behavior and social skills at home and in the community.
- Collaborate with the multidisciplinary team to ensure seamless care and support.
Requirements:
- High school diploma or equivalent.
- Valid driver's license, with reliable transportation and auto insurance.
