Title : Physical Therapist
Territory : Mid-Coast Region- Bowdoin, Topsham, Brunswick, Durham, Lisbon, Sabattus, Wales
What You’ll Do :
- Provide physical therapy services to clients according to standards of practice outlined by the American Physical Therapy Association. Services may include screening, evaluation, direct service, consultation and / or instruction.
- Demonstrate knowledge and case management skills to coordinate client home care needs in collaboration with client, family, physician, other providers and pay sources towards outcomes promoting client independence and well-being.
- Develop, implement, coordinate, and revise a plan of care that incorporates goals consistent with the client’s age, condition, and needs.
- Implement client care plan activities based on treatment goals towards independent living for the client / family.
