Job Overview
The post holder will be based at Monnow Vale Health & Social Care Facility, Integrated Service Hub for Central Monmouthshire, working
alongside health & social care colleagues in a well-established community based Integrated Team. The post holder will lead on the
development & delivery of a specialist falls service for Tier 3 Complex Fallers, engaging the wider MDT, and working alongside existing
qualified and support staff, and support delivery of a physiotherapy service to Tier 2 Fallers requiring Reablement input.
The postholder will additionally manage a general community-based caseload as required
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
