Job Overview
The post holder will be an autonomous practitioner, working within codes of practice and will manage a varied and complex caseload providing
highly skilled Occupational Therapy intervention for Monmouthshire residents who are in their own home and in-patients at Nevill Hall Hospital, Abergavenny. To facilitate the maintenance and improvement in their wellbeing to complete their activities of daily living, through Reablement and any other OT Intervention that may be required. Peers or external reference points may provide guidance.
The post holder will work in conjunction with other Band 6 Occupational therapists in this role and provide ongoing input to those individuals
who live in Abergavenny area alongside the other multidisciplinary workers in integrated services and provide OT assessment and intervention input into Nevill Hall Hospital for patients who live across Monmouthshire to ensure timely discharge. They will also be expected to complete generic tasks as the lead worker on the case such as commissioning and reviewing long term care and support. The post holder will be expected to work towards the development of experience within his or her own clinical specialism.

Main duties of the job

- To plan and prioritise own patient workload.
- To assess and address occupational therapy performance skills in the areas of mobility, sensory deficits, intrapersonal skills, intrapersonal
- To assess and interpret patient conditions and provide occupational therapy interventions in cases presenting a high degree of complexity
- Communicates condition related information, sensitive information to patients and families which requires empathy & reassurance.
- To identify and order equipment needed for patients to maintain their independence within their home environment.
- To maintain records and documentation required by work settings in accordance with the Monmouthshire Integrated Service and COT
- To attend and participate in wards rounds/case conferences / MDTs.
- To adhere to the COT code of professional conduct and ethics including confidentiality.
- To assess for and manage risks.
- To actively develop and maintain close working relationships with colleagues across disciplines and agencies. This will include agencies

- Degree or postgraduate Diploma in Occupational Therapy HCPC Registratio
- Plus experience to post-graduate equivalent diploma level in clinical area, eg, perception, cognition or equivalent demonstrable knowledge and experience
- Practice Placement Educator
- Supervision experience of junior/unregistered staff or students
