We are looking for a Licensed Vocal Nurse who is looking for an opportunity to grow with our company within the following counties, San Francisco County and San Mateo County. We are seeking compassionate and dedicated Licensed Vocational Nurse to join our home healthcare team. The ideal candidate will have a strong background in nursing, focusing on providing the best patient care
Essential Job Functions
Provides direct patient care as defined in California's Nurse Practice Act.
Implements plan of care initiated by the registered nurse.
Provides accurate and timely documentation consistent with the plan of care.
Assesses and provides patient and family / caregiver education and information pertinent to diagnosis and plan of care.
Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and / or Clinical Supervisor / Nursing Supervisor.
Uses equipment and supplies effectively and efficiently.
Participates in personal and professional growth and development.
Performs other duties as assigned by the registered nurse.
Core skills in patient care, physiology knowledge, and nursing
Proficiency in EMR and EHR systems
Ability to work with patients in San Francisco and San Mateo Counties.
Knowledge of HIPAA regulations and patient assessment techniques
Compensation : $55-$70 Per Diem Per Visit
