What are the responsibilities and job description for the Substance Abuse Counselor position at Angel's Light Halfway House?
Reports to: Facility Director
Job Responsibilities:
- Counsel patients, individually and in group sessions, to assist in overcoming addictions, adjusting to life, and making changes.
- Complete and maintain accurate records and reports regarding the patients' histories and progress, services provided, and other required information.
- Develop patient’s treatment plans based on established policies and regulations, clinical experience, and patients’ histories.
- Review and evaluate patients' progress in relation to measurable goals described in treatment and care plans.
- Interview patients, review records, and confer with other professionals in order to evaluate individuals' mental and physical condition, and to determine their suitability for participation in a specific program.
- Intervene as an advocate for patients in order to resolve emergency problems in crisis situations; Deescalate and address situations professionally.
- Provide patients or family members with information about addiction issues and about available services and programs, making appropriate referrals when necessary.
- Modify treatment plans to comply with changes in patient’s status when applicable.
- Coordinate counseling efforts with mental health professionals and other health professionals such as doctors, nurses, and social workers.
- Attend training sessions in order to increase knowledge and skills.
- Plan and implement follow-up and aftercare programs for patients to be discharged from treatment programs.
- Counsel family members to assist them in understanding, dealing with, and supporting clients or patients.
- Participate in treatment team and staff meetings.
- Coordinate activities with courts, probation officers, community services and other post-treatment agencies.
- Confer with family members or others close to patients in order to keep them informed of treatment planning and progress.
- Follow progress of discharged patients in order to determine effectiveness of treatments.
- Develop, implement, and evaluate public education, prevention, and health promotion programs, working in collaboration with organizations, institutions, and communities.
Job Skills and Qualifications:
- Bachelor’s degree in counseling or a related field
- State licensing or certification
- Experience in individual and group counseling
- Knowledge of DDAP, Carelon, & CCBHO Standards
- Understanding of ASAM and DSM-V diagnoses
- Excellent written and verbal communication, active listening, time management, organizational skills
- Strong social awareness and compassion for challenging, sensitive interactions
Job Type: Full-time
Pay: $22.00 - $25.00 per hour
Expected hours: 40 per week
