What are the responsibilities and job description for the Outreach Community Advocate position at Angel View Inc?
Position Overview
In an effort to increase the visibility of the Angel View Outreach program and increase the number of active participants, an Outreach Community Advocate will consistently participate in events throughout the Riverside County. The Community Advocate works directly under Program Director.
Case Manager Responsibilities
· Intake screening
· Maintain and provide weekly & monthly reports
· Cal-Works applicants/clients
· referrals/resources
· This individual manages a caseload of moderate size and complexity normally involving a limited number of disability areas.
· Interviews clients, their families, and other responsible individuals; assists in completing the application for services; collects basic data and obtains appropriate additional information from other agencies; participates on the inter-disciplinary team to review each case.
· Prepares or dictates case progress notes, social summaries, and other reports concerning the client's case.
· Creates and revises systems and procedures by analyzing operating practices, recordkeeping systems, forms control, office layout, and implementing changes.
· Resolves administrative problems by coordinating preparation of reports, analyzing data, and identifying solutions.
· Provides information by answering questions and requests.
· Completes operational requirements by scheduling and conducting initial intakes for outreach clients.
· Maintains professional and technical knowledge by attending educational workshops; establishing personal networks, and participating in community outreach events.
· Contributes to team effort by accomplishing related results as needed.
*Occasionally will be required to attend to events during the weekend.
Required Skills and Abilities
- Prior Community Outreach experience
- Ability to carry heavy case load
- Ability to prioritize and organize workload
- Willingness and ability to learn outreach database system
- Ability to communicate, and work with wide variety of people, across cultures and sub-cultures, in the community
- Skilled in Excel
- available to work some weekends and after normal work hours (with advance notice)
- MUST have own vehicle
- MUST willing to travel
Education and Qualifications
Associate Degree or HS Diploma with two years’ experience
MUST BE BILINGUAL English/Spanish
Good listening and problem-solving skills
Have the ability to work as a team and strong networking skills to build community partnerships
Excellent oral and written communication skills; Computer and word processing skills
Must have current CA driver license, clean driving record and reliable transportation to provide services listed above
Job Type: Full-time
Pay: $20.00 per hour
