Description
Follows established standards of nursing practice and Angela Hospice Home Care, Inc. policies and procedures
Communicates with other members of the Interdisciplinary Group (IDG) and implements appropriate follow-up action as needed
Adheres to the Patient Rights Program
Performs comprehensive assessment of assigned patients in a timely manner on admission and as required
Documents assessment, care plans and care provided according to Angela Hospice Home Care, Inc, policies and procedures
Reports changes in patient conditions to responsible party and takes follow-up action as necessary
Performs basic nursing skills in a knowledgeable and competent manner that provides for continuity of care
Administers and documents medications/treatments as ordered by the physician and according to facility policies/procedures
Instructs patients/families regarding medications, treatments and procedures, disease processes, self-care techniques, end of life care, and processes for dealing with issues of ethical concern
Initiates appropriate nursing services as indicated
Assesses ability of patient caregiver to meet patient needs
Applies Angela Hospice Home Care, Inc admission and re-certification criteria to ensure that patients meet hospice eligibility are placed at the appropriate level of care
Uses established confidential communication channels to communicate pertinent information
Assesses comfort, skill and pain levels of patient at each visit
Communicates patient/family concerns and initiates corrective action
Accepts on-call as assigned and as necessary
Provides clinical supervision to hospice aides and volunteers, as necessary
Communicates and reports clinical findings to insurance case managers, as requested
Makes periodic phone calls to referring physician as patient’s condition changes
Identifies safety hazards and corrects
Assumes responsibility for compliance with federal, state and local regulations
Presents an appropriate, professional appearance in accordance with established policy
Demonstrates dependability and punctuality
Accepts assigned duties in a cooperative manner
Works cooperatively with patients, families, nursing staff physicians, consultant personnel and ancillary service providers
Continues further educational goals by attending workshops, seminars and studies
Performs other duties as directed by the Home Care Clinical Supervisor
Qualifications
- Previous RN experience required, 1 year preferred
- Current license in the State of Michigan required
- Hospice experience preferred
