We are a family oriented Home Health Agency looking for compassionate Physical Therapists to join our team. We are a small company looking to expand our coverage throughout the San Francisco Bay Area. Our passion for providing the best healthcare to our patients is the driving force behind our need to expand.
- Provides physical therapy services to patients according to a written physician's plan of care.
- Initiates physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences.
- May train patient in the use of prosthetic device.
- Identifies patient and family/caregiver needs for other home health services and refers as necessary.
- Provides physical therapy consultation to home families/caregivers when indicated.
- Supervises Physical Therapy Assistants according to organization policy and state regulations.
- This position's location is Per Piem Rate per visit $130 - $150 Solano County
Minimum Qualifications:
- Licensed Physical Therapist (PT)
- Newly Graduated Physical Therapist (PT) are welcome
- Preferred: 2 years of physical therapy experience or combination of experience and education
- Experience Preferred: 1 year of home health experience
- Current CPR card for Healthcare Providers
- Must be a licensed driver with an automobile that is insured and in accordance with state/or organization requirements and is in good working order
- Ability to travel to see patients in their home
- Demonstrates good verbal and written communication, and organization skills
Salary : $130 - $150
