We are looking for a Licensed Vocal Nurse who is looking for an opportunity to grow with our company within the following counties, San Francisco County and San Mateo County . We are seeking compassionate and dedicated Licensed Vocational Nurse to join our home healthcare team. The ideal candidate will have a strong background in nursing, focusing on providing the best patient care
Essential Job Functions
- Provides direct patient care as defined in California's Nurse Practice Act.
- Implements plan of care initiated by the registered nurse.
- Provides accurate and timely documentation consistent with the plan of care.
- Assesses and provides patient and family / caregiver education and information pertinent to diagnosis and plan of care.
- Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and / or Clinical Supervisor / Nursing Supervisor.
- Uses equipment and supplies effectively and efficiently.
- Participates in personal and professional growth and development.
- Performs other duties as assigned by the registered nurse.
- Core skills in patient care, physiology knowledge, and nursing
- Proficiency in EMR and EHR systems
- Ability to work with patients in San Francisco and San Mateo Counties .
- Knowledge of HIPAA regulations and patient assessment techniques
Compensation : $55-$70 Per Diem Per Visit
Salary : $55 - $70
