JOB TITLE: HOME HEALTH AIDE
REPORTS TO: Senior Care Team Coordinator AND Director of Nursing
The certified Home Health Aide is a non-licensed member of the home health care team who assists the client with the task of daily living as outlined in the written health care plan that is established by a Registered Nurse and is kept in the client's home. Home Health Aides are supervised by the Senior Care Team Coordinator at Angelic Home Care Solutions (hereby referred to as “agency”).
QUALIFICATIONS
1. Be at least 18 years of age.
2. Complete the agency's employment and certification process and successfully pass a criminal background check and drug screening.
3. A minimum of 1 year experience with geriatric patients, especially Dementia and Alzheimer's patients, or individuals with terminal illnesses/disabilities is preferred.
4. Be physically able to perform the duties outlined in this job description.
5. Current CPR/First Aid certification
6. Exhibit mature responsible behavior and adhere to guidelines set regarding patient confidentiality.
7. Be able to read, write, understand and speak English.
8. Be able to follow directions and accurately report any changes in a client's condition to the Registered Nurse.
9. Have reliable transportation to and from assignments.
ESSENTIAL FUNCTIONS
1. Record information pertinent to the client's care.
2. Remind clients to take medication at the appropriate time.
3. Plan and prepare meals according to the client's specific dietary requirements and, if necessary, feed the client.
4. Assist the client with therapy exercises under the direction of a Registered Therapist.
5. Assist with or perform personal care, including hair care, oral hygiene, bathing, grooming, and dressing.
6. Assist with ambulation with or without mechanical aids.
7. Assist with routine bodily functions, including toileting.
8. Maintain a safe, clean and healthy environment by performing light housekeeping such as changing bed linens, dusting and vacuuming, cleaning kitchen and bathroom, and laundry.
9. Perform other housekeeping tasks as indicated in the Client Care Plan.
10. Provide companionship and stimulation for clients, including reading, walks, etc., as appropriate, and accompany the client to the doctor or other appointments.
11. Perform errands such as grocery shopping, picking up prescriptions or other errands as needed.
12. Take clients vital signs and record at the start and end of shifts.
13. Observe and report any changes in the client's condition to the agency's Registered Nurse.
14. Prepare daily records and submit them to the office by the required deadline.
Home Health Aide MAY NOT:
1. Administer or pour medication, as mandated by law.
2. Cut/trim the nails of any patient.
3. Perform any type of heavy housework such as cleaning stoves with oven cleaners, moving heavy furniture or appliances, shoveling snow etc.
4. Post on social media while wearing agency logos OR at patient homes. Anyone found representing the agency brand or logo without our direct consent will be subject to legal and civil penalties.
Job Types: Part-time, PRN, Contract
Pay: $16.75 - $17.00 per hour
Expected hours: No less than 20 per week
