Pay: DOE
$90 per visit, plus mileage - Assessment/Reassessment
Organizes and conducts medically prescribed physical therapy programs to restore function, prevent disability following disease, injury, or loss of a body part, and helps patients reach maximum performance levels. Evaluates, records and reports on patient progress. In addition, physical therapy is required to perform comprehensive at-home evaluations and maintain an open line of communication with patients, family members and caregivers.
QUALIFICATIONS:
- Graduate of an accredited school of Physical Therapy
- Successful completion of Physical Therapy Program approved by the American
Physical Therapy Association
- NM State Physical Therapist license
- 1 year experience working in home health, preferred
- CPR and first aid certification required
SKILLS/ABILITIES:
- Excellent communication and time-management skills
- Demonstrated ability to document accurately using charting software
- Excellent communication skills
- Must be detail oriented, flexible, and organized
- Must be able to maintain confidentiality regarding patient, employee, and company proprietary information, and
- Must have the ability to relate professionally and positively and work cooperatively with patients, families, and other employees at all levels
ESSENTIAL RESPONSIBILITIES:
- Assesses clients’ needs and develops plans for therapy that meets identified needs.
- Administers and performs therapeutic interventions with clients.
- Examines patients' medical histories, testing and measuring their strength, range of motion, balance, coordination, posture, muscle performance, respiration, and motor function
- Meets the patient’s goals and needs and provides quality care by assessing and interpreting evaluations and test results, determining treatment plans in consultation with physicians, and by prescription.
- Maintains patients’ self-esteem, privacy, and confidentiality of personal information.
- Provides patients with opportunities for independence and choice consistent with their care plans.
- Evaluates effects of therapy treatments by observing, charting, and evaluating patient’s progress, providing medically necessary treatment plan and modifications
- Notifies physician of changes, processes orders.
- Documents patient care services by charting in patient and department records according to accepted regulatory, corporate, and professional guidelines. Records daily treatment charges per corporate procedures.
- Provides direct consultation with physician and/or interdisciplinary team members; receives and shares information.
- Provides training on programs, adaptive equipment, and other interventions.
- Utilize and supervise therapy assistants as outlined by Federal and State regulations and documents appropriately in the clinical record. Utilizes appropriate bag techniques and standard precautions.
- Follows infection control and safety control standards when using equipment
