Job Description:
The primary function of this position is to administer skilled nursing care to clients of all ages in their place of residence, under the supervision of a registered nurse. This includes coordinating care with the interdisciplinary team, patient/family, and referring agency.
Required Skills and Qualifications:
- Graduate of an accredited school of practical/vocational nursing
- Current state license as an LPN
- Two years' experience as an LPN, home health preferred
