The Home Health Scheduler plays a key role in ensuring timely and efficient coordination of home health services for patients. This position involves scheduling, intake, and ensuring that all necessary resources are in place to provide high-quality, continuous care. The Scheduler will report to the Director of Operations and work closely with the Director of Nursing, field staff, and management to optimize care delivery and ensure compliance with scheduling protocols.
Key Responsibilities:
- Schedule and Coordinate Appointments: Manage and schedule patient visits for home health staff, including nurses, therapists, and home health aides. Ensure that appointments are booked efficiently and according to patient needs and availability of resources.
- Communication with Patients and Care Team: Serve as a primary point of contact for patients and caregivers to confirm and adjust appointments. Communicate any scheduling changes, cancellations, or updates to patients and the healthcare team in a timely manner.
- Maintain Accurate Scheduling Records: Keep detailed and accurate records of patient appointments, visit histories, and provider schedules. Update patient files as necessary and ensure that all relevant information is available to the care team.
- Resolve Scheduling Conflicts: Identify and resolve scheduling conflicts promptly, adjusting visits as needed to meet both patient and staff requirements. Prioritize urgent or emergency visits as necessary.
- Monitor and Track Patient Progress: Monitor the status of patient care and ensure that follow-up appointments are scheduled in a timely manner. Track visit frequency and adjust care schedules according to changes in patient needs or provider availability.
- Support Compliance with Regulations: Ensure all scheduling practices comply with applicable healthcare regulations, including HIPAA and patient privacy requirements. Keep track of any necessary documentation for audits and regulatory reviews.
- Collaborate with Healthcare Providers: Work closely with clinical staff to understand patient care needs and update the care team on patient status or any changes in the care plan.
- Handle Phone and Email Inquiries: Respond to phone calls, emails, and other inquiries related to scheduling and appointment requests. Provide excellent customer service to patients, caregivers, and staff.
Qualifications/Education:
- High school diploma or equivalent required; Associate's or Bachelor’s degree in healthcare administration, business, or a related field preferred.
Experience:
- Previous experience in a healthcare setting, preferably in home health or a similar field, is strongly preferred.
- Prior experience with medical scheduling software or electronic health records (EHR) is an advantage.
Skills and Competencies:
- Strong organizational skills with the ability to manage multiple tasks and prioritize effectively.
- Excellent communication skills, both verbal and written.
- Attention to detail and accuracy in recordkeeping.

Work Environment:
- This is a full-time, office-based position with occasional remote work options depending on company policy.
- The position may require occasional evening or weekend hours based on patient needs.
