Job Description
The Registered Nurse (RN) Case Manager coordinates all aspects of patient care, interacting with other medical disciplines as needed. Key responsibilities include:
- Performing admission visits to patients in their homes
- Determining patient eligibility for home care services
- Developing and implementing individualized plans of care
Requirements: Graduate of an accredited diploma, Associate, or Baccalaureate School of Nursing.
