A Children’s Care Coordinator responsibilities for below:
- Draft the Plan of Care for each member.
- Oversee that Plan of Care and the coordination of services.
- Support and encourage members in actively participating in reaching the goals set forth in their Plan of Care.
- Work closely with the Clinical Team Lead for Children, the Nurse Care Manager, and the Family/Youth Support Specialist.
- Encourage and support members connection with Family/Youth Support Specialist.
- Identify and link member with community resources.
- Facilitate behavioral and medical referrals.
- Collaborate with PCP, med providers and schools to support with facilitation of care.
- Provide ongoing case management care surrounding the member’s integrated physical and behavioral health needs
- Complete other duties as assigned.
The candidate must have excellent communication skills, a positive and outgoing personality, be motivated to make a change in our community and represent the agency and its mission in an engaging fashion.
Children’s Health Home Coordinator – shall be an individual who has a minimum of a Bachelor’s Degree from an accredited four (4) year institution of higher learning, with specialization in psychology, mental health and human services, behavioral health, behavioral sciences, social work, human development, special education, counseling, rehabilitation, sociology, nursing, or closely related field; OR who has a Bachelor’s Degree from an accredited four (4) year educational institution
in an unrelated field and at least one (1) year of full-time equivalent relevant human services experience; OR a who has Master’s Degree in social work, education, psychology, counseling, nursing, or closely related field from an accredited graduate school; OR who has been employed since August 1, 2009 as a case manager providing services under Chapter II, Section 13 of the MaineCare Benefits Manual.

Pay: $21.50 - $25.00 per hour
