Job Summary
The Intake Coordinator plays a crucial role in the healthcare setting, acting as the first point of contact for patients and ensuring a smooth onboarding process. This position requires strong organizational skills, attention to detail, and a comprehensive understanding of Behavioral Health Home Program. The Intake Coordinator will be responsible for scheduling with newly referred clients and completing the opening process.
Responsibilities
- Contacting newly referred clients, scheduling to meet with them in their homes, the community or office.
- Completing required opening paperwork.
- Completing necessary releases.
- Completing initial treatment plan and comprehensive assessment.
- Maintain patient files in accordance with regulations, ensuring confidentiality and security of sensitive information.
- Collaborate with team members to improve intake processes and enhance patient experience.
- Working with our Behavioral Health Team to best assign new clients to a Care Coordinator.
Qualifications
- Valid MHRT/C
- 2 years working in Care Coordination/ Case Management (preferred)
- Knowledge of HIPAA regulations and compliance standards is essential.
- Proficiency in using electronic health record systems is an advantage.
- Strong organizational skills with the ability to manage multiple tasks efficiently.
- Excellent communication skills, both verbal and written, to interact effectively with patients and healthcare professionals.
- Ability to work collaboratively within a team environment while maintaining a high level of professionalism.

Job Type: Full-time
Pay: $23.00 - $25.00 per hour
