Job Summary:
This position greets patients, assembles, interprets, analyzes, and enters patient financial and demographic data in the EHR system. Provides general office support with a variety of clerical activities and related tasks. Will work closely with medical providers and clinical staff and support their needs as directed
Reports to: Director of Outpatient Services; Urgent Care Team Lead
Hourly range: $18.50 - $32.76 (posted hourly range excludes overtime and bonuses which are not guaranteed)
Essential Job Functions:
· Greets patients, families, and guests promptly and courteously as the patient arrives at the clinic.
· Accurately registers all patients including collection of insurance information, demographics and full legal names. *verify all new demographics on existing patients especially phone numbers and billing address*. Obtains full verification of insurance.
· Provides interpretation of forms to be signed upon admission including patient rights and living will questions.
· Reviews all patient forms for completeness, accuracy, and appropriate signatures.
· Collects self-payments and/or co-pays according to established guidelines and accurately notes appropriate patient accounts.
· Responsible for accurately scanning all patient documents into the appropriate locations in the EHR system.
· Answers the telephone, provides requested information or takes messages, and directs caller to appropriate personnel.
· Transfers/coordinates phone calls/ checks voicemail and returns calls routinely throughout the day.
· Checks Urgent Care faxes throughout the day and disperses them to the appropriate staff member.
· Assist providers in contacting other medical professionals for consultations.
· Make appointments and send referrals for patients at various clinics as directed.
· Provide clerical assistance during patient transfers including- printing, copying, and faxing of medical records, assisting nurse with transfer packets, and assisting with coordination of transport, and accepting hospital and/or provider.
· Assists clinical staff as needed while remaining within scope of practice including:
o Assisting with medical emergencies.
o Direct and transfer patients to rooms as directed by clinical staff.
o Rooming and triaging patients, obtaining vital signs and health information.
o Executing orders as given by provider and directed by clinical staff.
o Assists clinical staff in patient care duties, as needed, including patients’ vital signs, EKGs, collection and testing of labs, dressing of wounds, and splint applications.
o Provide patient and patient family education on optimal treatment plans. Patient phone call follow ups on previous visits and assistance with patient phone call clinical questions.
· Collaboration between various clinics for continuity of care including calling report to Emergency Department or other necessary clinic nurse for transfer of care.
· Assists with point of care labs and follow up on send out laboratory results.
· Maintains supplies, equipment, stocking and sterilization of instruments. Maintains examination rooms, including minor cleaning, stocking of linens and removal of dirty supplies and linens.
· Direct and transfer patients to rooms as directed by clinical staff.
· Use of strong communication skills, reporting all duties to members of the team to create an efficient work environment.
· Practices caution and uses good judgment in relaying messages and giving information to patients, families, and visitors.
· Directs any patient, family, or visitor complaints to the appropriate person for prompt resolution (to the nurse first, then team lead, then director of outpatient services as applicable).
· Assists with front line initial assessment of patient and initiates rapid response to clinical staff as needed.
· Applies the principles and values of customer service and continuous quality improvement while performing day to day activities of the position
· Maintains and protects confidentiality regarding all aspects of patient care, personnel and strategic issues. Adheres to Confidentiality Policy.
· Maintains positive working relationships and fosters a cooperative work environment. Adheres to the hospitals Guidelines for Appropriate Conduct in the HR Manual.
· Displays honesty and mutual respect when communicating with peers and other departments.
· Follows through on problems that may compromise effective job performance by using appropriate chain of command

Minimum Requirements
Education: High School Diploma or the equivalent
Experience, Training, Knowledge: Minimum of one (1) to two (2) years’ experience as a clerk, CNA, or MA in a medical setting or on the job training.
Preferred Certification/Licensure: CNA or MA
Physical Requirements:
This position requires lifting, carrying, pushing, and/or pulling up to ten (10) pounds. Stooping, kneeling, crouching, reaching, grasping, handling, and balancing 5 or more hours per day. Sensory and communicative activities including: hearing, seeing, and speaking to be able to carry out essential job functions.
Working Conditions/Environmental Exposures:
Environmental hazards such as chemicals, solvents, needles, needle containers, sharps, blood, body fluids, various infectious/communicable diseases, infectious medical waste, radiation, and/or radioactive materials and/or radioactive waste, proximity to moving mechanical parts. Protective clothing/equipment may be necessary.

ASH IMMUNIZATIONS AND VACCINATIONS REQUIREMENTS – Aligns with CDC’s Advisory Committee on Immunization Practices (ACIP) for Healthcare Personnel
Hepatitis B- Lab evidence of immunity (titer) to Hepatitis B or three doses Hep B vaccine at appropriate intervals
Influenza- documentation of current seasonal flu vaccine (mandatory from November 1st through March 31st each year)
Measles, Mumps, and Rubella - Lab evidence of immunity (titer) to Measles, Mumps, and Rubella or 2 doses of MMR Vaccination
Varicella (chickenpox) - Lab evidence of immunity (titer) to Varicella or 2 doses of Varicella Vaccine
Tetanus, diphtheria, pertussis (Tdap) - documented proof of immunization
