Reports To: Agency Manager, IHSS Healthcare Professional, Supervisor
Supervises: NONE
The health maintenance attendant shall provide personal and health maintenance care as instructed.
DUTIES AND RESPONSIBILITIES include the following.
ESSENTIAL DUTIES ARE:
● Provision of health maintenance tasks per care plan and only as instructed for assigned consumer,
which may include traditional CNA level personal care or basic Nursing tasks;
● Assists the client with or provides personal care as assigned by the plan of care. Personal care duties
include, but may not be limited to: bathing, care of hair, skin, and mouth, toileting, dressing, transfer,
ambulation, medication reminding.
● Performs light housekeeping duties suffi cient to keep the client safe in the home. Housekeeping duties
include: making and changing bed, cleaning bathroom, personal laundry, cleaning kitchen area, and
shopping and errands.
● Prepares and serves meals.
● Documents activities performed in the client’s home at the time of the visit.
● Documents client concerns, and client diffi culties, and reports the same to supervisor or manager.
● Provides services when scheduled.
● Communicates promptly to agency and client any schedule change.
● Completes and submits required paperwork accurately and on time.
● Accomplishes job responsibilities in a timely, safe, and professional manner.
● Participates in client case conferences according to agency and procedures.
● Represents the Agency in a loyal manner and dresses appropriate to job duties
● Report any changes in consumer to IHSS Healthcare Professional.
OTHER DUTIES: Other duties as assigned.
EDUCATION and/or EXPERIENCE:
● High school diploma preferred, GED accepted.
● Experience of a minimum of two (2) years providing personal care preferred, six (6) months
accepted; prefer one (1) year employment history with licensed agency experience.
ABILITIES, SKILLS AND KNOWLEDGE:
● Ability to read and write and understand English language communication.

● Ability to send/receive emails.
● Ability to use computer for communication and documentation purposes preferred.
● Ability to use judgment to maintain confi dential information.
● Thorough knowledge of home health care practice, communication skills, and case management.
● Ability to establish and maintain effective working relationships with fellow employees, the public,
and supervisors.
SPECIAL REQUIREMENTS:
● At least 18 years of age;
● Must have no history of a nurse or nurse aide license or certification suspended or revoked or an
application for the same denied.
● Current TB screening upon hire and annually thereafter;
● Possession of a valid Colorado Driver's License with reliable transportation and applicable car
insurance, or statement of alternate reliable and available transportation.
PHYSICAL REQUIREMENTS:
● Ability to regularly sit, stand, stoop, bend, kneel, walk, balance, crouch, crawl, push, pull, reach with
hands and arms and talk and hear.
● Ability to frequently use hands to fi nger, handle, or feel.
● Ability to occasionally climb and use sense of smell.
● Ability to frequently lift and/or move up to 50 pounds and occasionally lift and/or move 100
pounds.
● Ability to perform the anticipated level of physical assist required to provide safe client care. If the
anticipated level of assist is greater than that which the employee can safely provide, the
supervisor is to be notifi ed immediately.
● Ability to use close vision, distance vision, color vision, peripheral vision, depth perception.
● Ability to adjust focus.
WORK ENVIRONMENT:
● The employee is frequently exposed to outside weather conditions.
● The work environment is highly variable, as care is provided in private homes.
● Lighting, acoustics, air quality, temperature and environmental hazards are not predictable.
● The employee is occasionally exposed to wet and/or humid conditions, household pets/animals,
fumes or airborne particles, and toxic or caustic chemicals.
● The noise level in the offi ce work environment is usually moderate.
TRAVEL:
● The employee is required to travel frequently to complete consumer visits.
