SUMMARYAssists physician and health care team members in understanding the significant social and emotional factors related to the health problems; participates in the development of the plan of treatment; prepares clinical and progress notes; works with the family; utilizes appropriate community resources; leads patient groups; and participates in discharge planning. The SW will be assigned to the Behavioral Health Unit and work with both the geriatric and adult populations.Qualifications include : ? Bachelor’s Degree in Social Work required, Master’s preferred.? License in Social Work required? Knowledge regarding community resources required.? One year of hospital / health care experience preferred.? Excellent communication skills and ability to work in a collaborative team approach required.
