What are the responsibilities and job description for the Chronic Disease Management RN Navigator position at AnMed?
The role of a Health Partner is to establish and maintain collaborative relationships with physicians, other health care providers, patients and their families to achieve desired patient outcomes throughout the continuum of care. It focuses on five primary areas: population management, patient self-management support, transitions in care, resource connection, and appropriate resource utilization.
Qualifications: Bachelor's Degree in Nursing with current SC nursing license. Minimum 5 years of chronic disease management experience (can be a combination of clinical experience). Willingness to obtain Certified Health Coach or disease management certification status as assigned. Have a current South Carolina motor vehicle license. Comfortable in a variety of settings, meeting patients, and working with a team. Demonstrates professional and effective communication skills. Willing to use personal transportation (mileage reimbursed) for home visits and other patient-centered travel such as joint physician visits. Familiarity with EHR, Microsoft Word and/or other similar computer software.
