What are the responsibilities and job description for the Clinical Pharmacist - Population Health position at AnMed?
Performs proactive direct patient care for targeted patient groups using a consistent and efficient approach that reflects a commitment to patient-focused care with the goal to improve medication therapy outcomes. The Population Health Clinical Pharmacist will promote and implement safe, rational and cost- effective drug therapy as evidenced by daily interaction with healthcare disciplines to positively influence medication therapy. The Population Health Clinical Pharmacist will demonstrate leadership and proficiency in patient specific medication use including managing and reporting of adverse drug events and the provision of comprehensive drug/disease information and education. Additionally this person may identify, develop, and assist with implementation of new services to meet the pharmacy needs of the patients and customers we serve.
