Job Description

Develops and coordinates the school services for patients in their school environment. Services as a resource to parents and schools about the specialneeds of patients affected by specific medical diagnoses and their rights within the school system. Coordinates the adaptation of classrooms and educational plans for patients by supporting the family and the school throughout the IEP/504 process.

Job Duties:

  • Participates in the coordination of service delivery with families and appropriate team members across the continuum. Makes appropriate referrals to team members and community resources.
  • Develops and implements programs with appropriate school district to provide services to students with specific medical conditions.
  • Coordinates individual education plans with the patient’s family, school and medical team. Assures that the patient’s educational potential and limitations, in relation to his or her illness, are effectively communicated to patient’s family, school and medical team.
  • Consults with families concerning the educational needs of patients and develops relationships with families that are caring, trusting, reliable and beneficial.
  • Develops programs and/or resources to educate school staff, administrators, and students about specific diagnoses and related needs. Stays current with education and research and implements research findings into practice.
  • Communicates with members of the disciplinary health care team, through formal and informal means of transmitting information, in order to provide optimal care of patients and families. Communicates through both conveying and responding to pertinent information. Assures that the patient’s educational needs are integrated into the interdisciplinary plan of care.
  • Advocates for and addresses the psycho-social/spiritual/developmental/cultural needs of the child and family throughout the continuum of care.
  • Participates in the team’s group problem-solving and program planning. Handles difficult situations pro-actively and collaboratively at the point of service. Provides leadership for the team in the delivery of family services.
  • Establishes annual goals and effectively works to meet these goals.
  • Assumes responsibility for professional development through participation in seminars, course work, and/or hospital educational programs. Develops and implements plan for necessary professional credentials, and maintenance of such credentials.
  • Networks with other local and national disease-related programs. Mentors interns and colleagues, as appropriate.
  • Maintains appropriate records and statistics. Orders and maintains materials for service and program needs.
  • 1Performs job functions adhering to service principals with customer service focus of innovation, service excellence and teamwork to provide the highest quality care and service to our patients, families, co-workers and others.

K.S.A.'s:

  • Requires knowledge of special education needs for grades K-12 normally acquired through the completion of master’s degree in education or a Licensed Social Worker with five years of experience working in the school environment.
  • Ability to think pro-actively, act creatively and take initiative appropriately.
  • Knowledge of special education policy and law.
  • Ability to successfully strategize working within school systems and coordinating statewide programming.
  • Knowledge of research methods for purpose of prospective studies.
  • Ability to communicate effectively, work as a member of a team and adapt to the changing health care environment is essential.

Pay Range

$54,600.00-$89,273.60 Salary

The pay range for this job indicates minimum and maximum targets for the position. Ranges are regularly reviewed to stay aligned with market conditions.
