POSITION QUALIFICATIONS:
- Have training and experience in health care and at least 1 year of supervisory or administrative/clinical experience in home health care or related health programs
- Exhibits leadership skills with the ability to work independently and as part of a team

The care coordinator I is responsible for the day-to-day patient care management of the organization, including but not limited to:
1. Assist in the development and implementation of individualized care plans for clients.
2. Coordinate and schedule caregiver assignments to ensure coverage and continuity of care. I will be with clients throughout their entire shift when coverage is not available.
3. Understand that I will be scheduled/available to work two weekends monthly as needed.
4. Participate in employee/contractor hiring, orientation, and ongoing training program. All orientation and training will be by the Agency’s policies, and a Staff Training Record will be maintained.
5. Monitor and evaluate caregiver performance to ensure quality of care. Provide ongoing training (i.e., in-services), guidance, and support for caregivers. Ensure that all client records are up-to-date and accurate.

6. Communicate effectively with clients, caregivers, and healthcare professionals to address any concerns or changes in care needs.
7. Monitor and document client progress, updating care plans as necessary.
8. Provide watchful oversight and supervision of client care with qualified staff to provide the required services. Conduct regular check-ins with clients and caregivers via home visits & telephone calls to ensure satisfaction and address any issues.
9. Organize/conduct staff meetings as needed.
10. Ensure the quality assurance program is ongoing by monthly periodic audits of client’s charts with supervisory visit with clients every 45 days.
11. Ensuring client quality and appropriateness with availability of services. To ensure appropriate staffing to meet clients’ service needs with providers who have the skills and competence to meet clients’ needs and to deliver the services and care advertised.
