Summary of Duties
Coordinate the functions of the Social Services department in accordance with regulatory guidelines and established policies and procedures. Assure that an on-going social services program is designed to meet the medically related psychosocial needs of each new resident. Assist the Admissions/Marketing team with inquiry intake, evaluation, and admission of new residents.
Essential Job Functions
- Interview residents or family members to obtain psychosocial information.
- Complete comprehensive assessments of the psychosocial needs of each resident.
- Arrange services/appointments for residents as needed, including transportation, optometry, and dental consults.
- Review and revise care plans and assessments as necessary, but at least quarterly.
- Review complaints and grievances made by the resident and/or family and make a written/oral report to the Executive Director indicating what action(s) were taken to resolve the complaint or grievance following established procedures.
- Make written and oral reports/recommendations to the Executive Director concerning the operation of the social services department.
- Chart informative and descriptive social services progress notes of the services provided and of resident’s response to the service.
- Place orders for equipment and supplies as necessary or as may be required.
- Complete MDS, RAPs and Care Plans.
- Develop and implement discharge plans in conjunction with the resident, family, and other disciplines.
- Assist the Admissions/Marketing team with inquiry intake, evaluation, and admission of new residents.
- Assist in the planning and implementation of facility and community events as directed by the marketing committee.
- Perform other duties or special projects as assigned.
Education
High School Diploma required. Bachelor’s degree in Social Work or comparable, relevant work experience preferred.
Experience
Two (2) years experience in long-term health care preferred.
