This role will start as part-time and possibly transition to full-time as patient census grows. Key Responsibilities:
- Provide medical care, palliative care, and manage chronic and terminal illnesses.
- Complete history and physical examinations by evaluating the patient's medical condition and health history.
- Review clinical information and coordinate and oversee patient care.
- Consult with the palliative care interdisciplinary team and other healthcare practitioners.
- Review and develop protocols for treatment.
- Coordinate patients' plan of care.

Position Qualifications:
- Graduation from an accredited School of Nursing and accredited Nurse Practitioner Program.
- Current licensure in State and CPR certification as required.
- Reliable Transportation & Auto Insurance.
- Preferred: certification in a specialist area such as Geriatrics or Hospice and Palliative Nursing
- Familiarity with Electronic Charting
- Excellent skill in observational, verbal, and written communication and problem solving
Anova Care Benefits:
- Salary: 125,000 for full time (position will start as part-time)
- Mileage reimbursement
Hours: Monday – Friday, weekends as needed. Shifts available include mostly daytime with evening or weekend shifts possible
