Job Summary:
We are seeking a dedicated and compassionate Hospice Chaplain to provide spiritual and emotional support to patients, families, and caregivers. The Chaplain will collaborate with the interdisciplinary care team to ensure holistic care is provided to meet the diverse spiritual needs of those we serve.
Key Responsibilities:
- Provide spiritual support to patients, families, and caregivers, respecting diverse faiths, beliefs, and cultural backgrounds
- Conduct spiritual assessments to identify patients and family needs and preferences
- Develop and deliver individualized spiritual care plans in coordination with the care team
- Offer counseling and emotional support during times of grief, loss, and transition
- Lead or participate in prayer, meditation, or religious ceremonies, as requested by patients or families
- Facilitate connections with community faith leaders and resources when appropriate
- Collaborate with the hospice interdisciplinary team to ensure holistic patient care
- Maintain accurate and timely documentation of services provided in accordance with hospice policies
- Provide bereavement support to families and caregivers as part of ongoing care
- Participate in team meetings, in-service training, and continuing education to stay informed about best practices in hospice chaplaincy
Employment Type: PRN transitioning to full time
Qualifications:
- Graduate of an accredited seminary or school of theology or appropriate certification in hospital or pastoral ministry
- Ordination or endorsement by a recognized faith group or denomination
- Minimum of 2 years experience as a chaplain/spiritual counselor
- Active patient contact within the past three years preferred
- Minimum of one unit of Clinical Pastoral Education (CPE); additional units preferred
- Experience providing spiritual care in a hospice, healthcare, or pastoral setting
- Strong interpersonal skills, cultural sensitivity, and the ability to work with individuals from diverse backgrounds
- Excellent listening, counseling, and communication skills
- Ability to handle emotionally challenging situations with compassion and professionalism
- Proficiency in maintaining confidential records and adhering to HIPAA guidelines
- Valid drivers license and reliable transportation to visit patients in their homes or care facilities.
Preferred Qualifications:
- Certification with a professional chaplaincy organization (e.g., APC, NACC, or ACPE).
- Experience providing bereavement support or grief counseling.
