Job Overview
We are seeking a dedicated and compassionate Case Manager to join our team. Responsibilities
- Conduct comprehensive assessments of clients' needs and develop tailored care plans.
- Provide crisis management and intervention services to clients in distress.
- Facilitate access to community resources, including healthcare, housing, and employment services.
- Offer addiction counseling and support for individuals struggling with substance use disorders.
- Mentor and supervise junior staff or interns as needed.
- Maintain accurate documentation of client interactions and progress in compliance with agency policies.
- Collaborate with multidisciplinary teams to coordinate care and services for clients.
- Engage in workforce development initiatives to empower clients in achieving their personal goals.
- Advocate for clients' rights and needs within various systems, including Medicare.
