What are the responsibilities and job description for the Case Manager/CBRS position at Answers LLC?
Answers LLC is looking for hard working, organized, and committed individuals for Case Managers/CBRS. Case managers work in collaboration with the client, family, and professional team to work towards goals outlined in the Individualized Treatment Plan. Case managers will link, and coordinate services and resources based on the client’s needs. CBRS Workers/Skill Building Paraprofessionals will provide services and teach mental and behavioral skills to clients based on the Individualized Treatment Plan.
Responsibilities
- Collaborate with other counselors, case managers, and staff to develop individualized treatment plans for clients.
- Develop relationships with clients and their families to promote recovery and resiliency
- Maintain accurate treatment records and be available.
- Attend monthly meetings with supervising therapist (or group) to review progress with clients' family members
- Complete required documentation in a timely manner
- Scheduling appointments and transportation for clients
- Coordination with other service providers
- Assistance with paperwork and insurance/benefit maintenance
- Providing other resources and referrals to adults and children to help meet goals.
- Meeting clients in their homes and community for skills development.
- Implementing skill building exercises in the home and community based on therapist recommendations.

- Strong interpersonal skills as an individual and in collaboration with others

- Computer literacy
- Ability to organize, prioritize and multi-task.
- Ability to analyze and problem solve.
- Experience with mental health population is preferred.
Requirements:
- Bachelor’s Degree Mandatory
- Must pass an enhanced criminal background check conducted by the Department of Health and Welfare
- Must be detail oriented with good organizational and communication skills to perform the required documentation, assessments, and interventions. Computer proficiency required.
- Must have a valid driver’s license, insurance, and acceptable driving record.
- Willing to travel to clientele's home and other locations.
Job Type: Full-time
Pay: $18.00 - $21.00 per hour
Expected hours: 25 – 45 per week
