Anthesis Position Description
OUTCOMES COORDINATOR
SHIFT: FULL-TIME
SCHEDULE: MONDAY through FRIDAY / May include evenings and weekends.
07:00AM - 04:00PM
PAY RANGE: $22.50hr to $23.50hr
Under the direction of the Outcomes Manager, the Outcomes Coordinator participates in the initial intake and coordinates with the person served and his/her circle of support to design and implement each individual’s Person Centered Service Plan (PCSP) objectives; identifying barriers to participant success in obtaining the PCSP objective(s) and solutions to problem areas for specified caseload of individuals. Assists the Outcomes Manager in assessing the program effectiveness in achieving individual PCSP objectives.
Principle Responsibilities:
- Ensure all job duties are performed in-line with and in support of the mission of Anthesis.
- Serve as a member of the Intake/Eligibility Committee. May conduct intake interviews and present summary of diagnostic information to the Intake Committee in the absence of the Intake Coordinator.
- In collaboration with the Outcomes Manager and Community Coaches, perform the 90-day assessment and convene a meeting with the participant and his/her circle of support.
- In conjunction with Outcomes Manager, provide orientation and training for program staff and participants following the Anthesis program description and agency policies.
- Work in partnership with the individual and his/her circle of support and program staff to design each participant’s PCSP objectives.
- Work in collaboration with the Outcomes Manager and Community Coaches to locate and develop appropriate integrated volunteer, work, educational, training, recreational, etc. activities within the local community that coincide with each person’s PCSP objectives. May fill in as direct support staff to meet the needs of participants.
- Collaborate with the Outcomes Manager in order to assist Community Coaches effectively plan out each participant’s daily/weekly activities based on each participant’s PCSP.
- Collaborate with the individual and his/her circle of support, the Director of IECBS/Outcomes Manager, and Community Coaches to evaluate participant progress on an annual and semi-annual basis.
- Interface with all Community Coaches to ensure they are providing a quality service, following through with the individual’s PCSP and partner to identify solutions to challenges.
- In collaboration with the Outcomes Manager, monitor daily/weekly contact notes and participant attendance as well as the attendance of Community Coaches.
- Implement program assignments as directed by Outcomes Manager and Director of IECBS.
- Maintain on-going communication to plan and implement participants’ preferred work activities/job tasks/volunteer sites with participants’ circle of support.
- Ensure that all participant incidents are reported promptly, accurately and in a timely manner.
- Assist the Outcomes Manager in maintaining ongoing relationships and follows-up with all Regional Center staff, community liaisons, funding sources and families to ensure participant success.
- Maintain program services in accordance with philosophies and objectives of Anthesis.
- Ensure compliance with the standards of referring agencies, certification, and Federal/State Departments of Labor, accreditation agency. Ensures that all organization activities and operations are carried out in compliance with local, state and federal regulations and laws governing business operations.
- Responsible for program compliance with all policies, systems, and procedures of Anthesis and standards of accreditation.
Essential Functions:
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.- Demonstrated ability to work well with persons with disabilities, elderly persons, families, representatives of residential facilities, and referring agency staff.
- Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions and decimals. Ability to compute rate, ratio and percent.
- Ability to speak and communicate effectively with program participants, staff members, care providers, family members, community members, etc.; a second language is desired but not necessary.
- Ability to read and interpret documents such as procedure/policy manuals, safety rules, routine reports and correspondence; ability to write routine reports and correspondence, including the ability to develop, compose and implement Individual Plans.
- Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists; ability to interpret a variety of instructions furnished in written, oral, diagram or schedule form.
- Ability to effectively deal with internal and external customers, some of whom will require high levels of patience, tact, diplomacy to defuse anger, collect accurate information and resolve conflicts.
Supportive Functions and Other Duties:
In addition to performance of the essential functions, this position may be required to perform a combination of the following supportive functions:- Ensures that responsibilities, authorities and accountability of all subordinates are defined and understood. Guides and assists program staff in various consumer and program issues, program plans, rights, advocacy, etc.
- Ensures personnel and consumer records are kept in accordance with confidentiality standards. Ensures consumer rights and confidentiality are respected by all staff.
- Conducts needs assessment and recommends new services when appropriate.
- Responsible for compliance with the safety standards of the agency. Helps to ensure safe and sanitary working and training conditions to protect the health and safety of all program participants, staff, and visitors of the agency. Reports safety violations to the Outcomes Manager.
- In conjunction with the Management Team, and Job Developer, acts as a contact/liaison between organization and business partners for contract work.
- Serves as liaison for specified consumer case load with residential care providers and family members, handling difficult situations as they arise.
- Assists in the achievement of the agency’s outcomes management systems for program.
- May participate in recruitment and training of specified program staff.
- Provides supportive supervision and technical assistance to staff and program participants related to program services.
- Assimilate into the Anthesis culture through understanding, supporting and participating in all elements of government regulations and policies.
- May serve as substitute Community Coach in the absence of staff; use of personal car may be required.
- Regular attendance in conformance with Anthesis standards, is essential to the successful performance of this position.
- Aptitude in handling stress and in resolving issues or conflicts; ability to interact with others no matter the position, department, program, are essential to the successful performance of this position.
- Attends training programs, which may include travel.
- Performs other duties as assigned.
Supervisory Responsibilities:
In conjunction with the Outcomes Manager, indirectly supervises community coaches and other identified staff. Carries out supervisory responsibilities in accordance with the organization’s policies and applicable laws.Education/Experience:
Must possess high school diploma or equivalent; Bachelor’s Degree from an accredited college or university in human services related field OR any combination of at least 3 years’ experience in human services delivery system, education, and training that demonstrates the ability to perform the duties of the position. Must have basic knowledge of and experience in, use of internet and email. Proficient in MS Office 2010 or later, particularly in Word, Excel, and PowerPoint. Database experience and Publisher a plus.Licenses or Certificates:
Current and valid California driver’s license with at least 3 years current driving experience and driving record that is acceptable to the organization’s insurance carrier. Must be able to provide proof of personal vehicle insurance coverage and vehicle registration. CPR/FA certification and/or ability to pass certification.Physical Requirements:
While performing the duties of this job, the employee is:- Regularly required to walk, sit, and talk and hear; regularly lift and/or move and/or push/pull program participants up to 25 pounds;
- Occasionally lift and/or move and/or push/pull program participants up 50 pounds;
- Frequently required to stand, use hands to finger, handle, or feel objects, tools, or controls; reach with hands/arms;

- Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception, and the ability to adjust focus.
Work Environment:
While performing the duties of this job, the employee is regularly exposed to air temperatures regulated by office thermostat and may be occasionally exposed to outside weather conditions, including heat, cold, rain, humidity, etc. The employee occasionally works near moving mechanical parts and the noise level in the work environment is usually moderate.Salary : $23 - $24
