PART-TIME DSP/COMMUNTIY COACH
 
SHIFT: PART-TIME
SCHEDULE: MONDAY throug FRIDAY AND POSSIBLY WEEKENDS
                      03:00pm - 10:00pm
 
PAY RANGE: $20.50/hr. to $21.50/hr.
 
Principle Responsibilities & Position Purpose:  Under the direction of the Outcomes Manager, the Community Coach supports adults with intellectual and developmental disabilities in natural settings on a daily basis in activities designed to promote vocational, social, physical and intellectual growth, as well as assist each client in skill development for community integrated employment.  Community Coaches are responsible for soliciting, following-up and securing job/volunteer leads for individuals and/or group placements; for implementing program curriculum; delivering individual and small group learning experiences to assist the clients in obtaining Person Centered Service Plans (PCPS); provide job-site/volunteer site training and intervention for Anthesis clients as assigned in community employment/volunteer settings to determine job training goals and ongoing success.
 
ESSENTIAL FUNCTIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.  
  • Responsible to ensure all job duties are performed in-line with and in support of the mission of Anthesis.
  • Provide means of daily transportation in personal vehicle to various locations and/or to meet clients at various locations.  Responsible for training clients on the use of public transportation, including types of transportation available, schedules, fees, etc.
  • Sets a good example of work rules and directives of community employer/volunteer program, displaying to the clients, the proper techniques, habits, attitudes for work.
  • Responsible for the work performance/volunteer task of client workers assigned to contract.
  • In conjunction with the Outcomes Manager, prepares and coordinates the daily, weekly and/or monthly schedules of planned activities and/or work schedules with each client assigned to group, through written and/or oral instructions.
  • Acts as a liaison with the employer and/or volunteer director, providing supports, consultation on employment/volunteer related issues for success of long term employment/volunteerism.
  • Aids Outcomes Manager and Case Managers in developing and implementing Individual Habilitation Service Plans (IHSP)/Individual Service Plans (ISP) for each consumer.
  • Responsible for monthly meetings with clients in IECBS program to discuss safety, codes of ethics, rules of conduct, etc.    
  • Responsible for the safety of all clients in personal care, including but not limited to, prompt reporting to the Outcomes Manager and/or Case Managers, of all injuries, serious events, safety related accidents, vehicle accidents, vehicle code violations, etc.

SUPPORTIVE FUNCTIONS and OTHER DUTIES

In addition to performance of the essential functions, this position may be required to perform a combination of the following supportive functions:
  • Ensures client rights are protected at all times and are respected by the community at large.
  • Responsible for maintaining confidentiality of client information carried throughout the day and kept in accordance with Anthesis’ confidentiality standards;
  • Will assess client’s skills and abilities and track progress on goals as a part of Outcomes data reporting.
  • Responsible for charting client’s progress for consumer folder; maintains accurate, individualized accounting of client’s attendance, working hours, volunteer hours, including documentation related to off-site billing for work performed; etc.  
  • Determines needed materials, parts, tools, equipment, etc., in order to complete jobs/tasks and keeps clients supplied with such items; sets up proper workstations and materials for the day’s activities and controls the waste of material and time. 
  • Responsible for intervention in behavior management problems in compliance with ISP’s and PCP’s. In conjunction with Outcomes Manager and Outcomes Coordinator, deals effectively with defiant behavior, morale issues, client workers’ and/or volunteers’ grievances or complaints, hygiene, grooming, etc. 
  • Responsible for safe operation of personal and/or agency vehicle when used during scope of work; responsible for accurate documentation of transportation/mileage records accordingly; responsible for neat and clean environment in personal or agency vehicle when used during scope of work.
  • Responsible for complying with safety standards of Anthesis.
  • Attends required department staff meetings/in-service classes as required by Anthesis; assists in discussion of off-site activities.
  • Other duties as assigned.

SPECIFIC JOB KNOWLEDGE, SKILL AND ABILITY
The individual must possess the following knowledge, skills and abilities and be able to explain and demonstrate that he/she can perform the essential functions of the job satisfactorily, with or without reasonable accommodation. 
  • Ability to work with adults with disabilities; training vocational skills by ways of explanations, demonstrations and hand-over-hand.
  • Must have the ability to appropriately handle stress and work effectively/diplomatically as a co-worker, community coach, and client advocate in order to defuse anger, collect accurate information and resolve conflict.
  • Ability to carry at least 3 clients in personal vehicle.  
  • Ability to work independently in the community in a variety of settings and assignments as needed to accommodate changing needs and/or requests by contracting employers, including hours of work and location.
  • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions and decimals. Ability to compute rate, ratio and percent.  
  • Ability to read, write, and speak English language enough to communicate with various members of management, clients, staff members, community members, etc.; ability to comprehend simple instructions and to write simple correspondence, such as charting client progress, issues, complaints, resolutions, etc.; demonstrated ability to communicate effectively both orally and in writing; to effectively present information in one-on-one and small group situations, to clients, customers, and employees of the organization.  A second language, including sign language is desired but not necessary.  
  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exisits; ability to interpret a variety of instructions furnished in written, oral, diagram or schedule form.
EDUCATION/EXPERIENCE
High school diploma or general education degree(GED); Associate of Arts Degree in a related field desired; sufficient experience with adults with intellectual and developmental disabilities to carry out duties required in above description; or any combination of education, training and experience that demonstrates the ability to perform the duties of the position.   
 
LICENSES OR CERTIFICATES
Personal vehicle required with current and valid California driver license. Three to five years driving experience with driving record that is acceptable to the organization’s insurance carrier. Must be able to provide proof of current vehicle insurance and registration. Current CPR/FA certification is desired and/or ability to pass certification.

  • Frequently required to stand, use hands to finger, handle, or feel objects, tools, or controls; reach with hands/arms;
  • Occasionally required to climb or balance; stoop, kneel, crouch, or crawl; taste or smell.
  • Specific vision abilities required by this job include various distances in vision.

While performing the duties of this job, the employee is regularly exposed to outside weather conditions including but not limited to, heat, cold rain, sun, humidity, etc.; may be exposed to air temperatures regulated by office thermostat.  Depending on the community setting, the employee may occasionally work near moving mechanical parts, may be exposed to loud noise and vibrations; may need to work in areas that are considered quiet spots, etc.
