Job Summary
We are seeking a highly skilled and compassionate LVN to join our team as a Patient Care Manager in a home setting. The ideal candidate will have experience in providing direct patient care, working collaboratively with interdisciplinary teams, and ensuring seamless care delivery.
Responsibilities
- Provide direct patient care and treatment under the supervision of a Registered Nurse
- Ensure high-quality patient experience and positive health outcomes by delivering the plan of care established by the RN Case Manager
- Prepare complete and timely clinical notes, coordinate care delivery with an interdisciplinary team (PT, OT, ST, MSW, and HHA)
Requirements
- Current CA Licensed Vocational Nurse (LVN) license
- Current IV Certification
- Current CPR card for Healthcare Providers
- 1 year of Licensed Vocational Nurse (LVN) experience
- Must be a licensed driver with an automobile that is insured in accordance with state and organization requirements and is in good working order
