Hospice Care
ANX Home Healthcare & Hospice Care also provides 24/7 hospice care for individuals with limited life expectancies or those no longer willing to seek aggressive medical treatment. Responsibilities
This role involves directing all daily patient referral and intake operations, ensuring compliance with state, federal, and Joint Commission referral/intake regulatory requirements. You will screen referred patients to meet home health eligibility criteria or admission criteria, direct the implementation of improved work methods and procedures, and ensure seamless transition of patients to home care by providing patient education and preparation for home care, plan of care initiation, and coordination of care with multiple service providers.
