- Ensures compliance with state, federal, and Joint Commission referral/intake regulatory requirements
- Screened referred patients to meet home health eligibility criteria or admission criteria
- Coordinates care with multiple service providers
- Maintains comprehensive working knowledge of ANX Home Healthcare contractual relationships and community resources
Requirements
- Minimum one year's experience as a home health LVN
- Ability to access electronic databases to obtain or enter pertinent patient information
- Knowledge of governmental regulations, Joint Commission standards, and private payer practices
