Monday-Friday (8 : 30 am - 5 : 30 pm)Position Summary of a Licensed Vocational Nurse (LVN) Hospice Clinical Coordinator :
The ideal candidate will have experience in hospice care and a strong background in nursing.We are seeking a highly organized and detail-oriented individual to join our team as a LVN Clinical Coordinator. The role allows for a balance between clinical and administrative responsibilities. This includes coordinating and supervising administrative staff, implementing and improving administrative processes, and ensuring efficient and effective operations across all departments.This is an exempt position and will report directly to the Director of Patient Care Services.Responsibilities :
The LVN Hospice Clinical Coordinator will be responsible for coordinating patient care, providing guidance and support to staff, and ensuring that all care provided meets the highest standards of quality and compassion. The Clinical Coordinator will also be responsible for conducting regular assessments and evaluations of staff performance, as well as participating in the development and implementation of care plans. Strong leadership and communication skills are essential for success in this role.Staff Coordination
Oversees the team schedule and coordination of services to insure coverage for all patients on the team.Reviews and assists in completion of payroll, assuring that employees have documented accurate time.Ensures that documentation are completed timely.Oversees staff and volunteer schedules, scheduling and territory assignments to assure that workload is distributed equitably and that all staff are able to meet overall productivity expectations.Reviews and approves payroll, assuring that employees have documented accurate time, mileage and additional expenses.Acts as a resource and mentor for staff re clinical issues, documentation, orientation of new hires.Participates in performance improvement activities to respond to service issues and challenges, and documentation issues.Monitors resources and utilization of clinical operations including but not limited to labor, pharmacy, medical supplies, equipment.Assures staff competence and performance levels through making field visits.Will supervise the office Admin staff ensuring clear lines of communication.Patient Care and Customer Service
Responsible for intake and coordination of referrals.Confident and comfortable in speaking with families about hospice services and the values and mission of ANX Hospice.Be able to provide hospice education to community partners.Participates in on-call rotation.Fills in and provides patient care within their scope of practice during times of need.Participates in the QAPI program by gathering data and assisting with data.Regularly visits facilities to assure care plan integration, customer satisfaction and to discuss criteria for determining appropriateness.Handle service issues together with the Account Executives.Staff, facility and documentation data collection may be collected remotely and does not need to be 100% present in the office.Interface with customers, patients, and families to promote patient and customer satisfaction. Creates partnership with referral partners to promote value and clinical expertise.Lead educational forums for both Home Health and Hospice and support community outreach programs to support branding and clinical differentiators.Must be able to adjust to changing dynamics of field and clinical operations due to customer and patient demands.Qualifications :
Experience in hospice settingProven experience in administrative management, preferably in a similar role.Strong leadership skills.Excellent organizational and time management abilities.Exceptional problem-solving and decision-making skills.Proficient in using office software and equipment.Excellent verbal and written communication skills.Ability to delegate and work well under pressure and meet deadlines.Strong attention to detail and accuracy.Ability to maintain confidentiality and handle sensitive information.Proficient in Google docs and graphs and sheets.Education
Current and valid LVN License to practice in California.
Salary : $90,000 - $104,000
