This role directs daily patient referral and intake operations, ensuring compliance with state, federal, and Joint Commission regulatory requirements. It involves screening patients to meet eligibility criteria, implementing improved work methods, and ensuring seamless transitions to home care through patient education and preparation, plan of care initiation, and coordination with multiple service providers.
Skills and Qualifications:
A minimum of one year's experience as a home health LVN is required, along with the ability to access electronic databases and knowledge of governmental regulations, Joint Commission standards, and private payer practices.
