Position Summary:
The CARE Social Worker provides comprehensive medical case management and supportive services to families of children and youth in Any Baby Can’s Comprehensive Advocacy and Resources for Empowerment (CARE) program. CARE serves children and youth with special health care needs (CYSHCN). This is a home visit and community-based program that provides a variety of support for families. The support services provided in this comprehensive case management program include development of individual service plans, planning transitions to school, transition to adulthood planning, emergency preparedness, establishing a medical home/addressing healthcare gaps, crisis intervention, and whatever is needed to address the individual client needs. Clients eligible for this program include all youth ages 0-20 with medical needs such as mental/behavioral health, intellectual and developmental disabilities, cancer, chronic illness, other medical conditions.
Key Responsibilities:
- Provide in-home case management in Travis, Williamson, Hays, Bastrop, Bell, Burnet, and Caldwell counties. Telehealth services are provided per family needs.
- Provide social and emotional support for parents of children with special healthcare needs in the form of Parent Cafes, group support or referrals.
- Maintain a caseload of 20-25 clients and provide case management services minimum 2x per month to meet program targets.
- Develop and maintain good rapport and trusting relationships with clients and their families.
- Process incoming referrals to determine eligibility, provide information and resources in the community as needed.
- Complete Intake, Family Needs Assessment, and develop an Individual Service Plan (ISP) in collaboration with the family and the youth for assigned clients.
- Provide school advocacy to ensure that educational needs are addressed aligned with the child's medical needs, including attending ARD meetings as needed.
- Attend weekly CARE team meetings and staffings for the purpose of further assessing the needs of each family and developing strategies. Attend agency required all staff meetings.
- Meet with CARE Program Supervisor 2x per month to talk about workload and cases.
- Assist clients with accessing respite services support to help the child’s caregiver get time for self-care and to prevent caregiver burnout.
- Complete case documentation within program and agency timelines.
- Attend and assist with agency respite events such as Parents’ Night Out, Back to School Movie Night, and Camp Grey Dove to ensure caregivers get breaks to prevent caregiver burnout.
- Facilitate referrals with community resources through advocacy and coordination and collaborate with other agencies to enhance service provisions and referrals.
- Other assignments may be added as needed or required.
Experience and Qualifications:
- Social Worker with a Master's degree in Social Work with licensure appropriate for their practice or Bachelor’s degree in Social Work with licensure appropriate for their practice and one year of experience providing case management services within community programs.
- LBSW or LMSW license is required.
- Fluent language skills in English both verbal and written modalities, required. Bilingual in Spanish preferred.
- Experience working with low-income and culturally diverse families.
- Familiarity working with children/families in medical settings, individuals with disabilities, and ability to handle crisis situations.
Special Considerations:
- This position requires periodic (on average about 10 days per year) availability outside the normal working hours to attend and facilitate program-related events, groups, and outreach opportunities. Flex time is offered when work is performed outside of agency business hours.
- Must have reliable transportation to travel within the assigned region; requires a valid driver’s license and current vehicle insurance.
- Flexible, able to work well under pressure and handle crisis situations.
- Ability to sit, stand, kneel as necessary to fulfill daily duties assigned.
- Ability to communicate effectively/clearly.
Salary : $55,000 - $57,000
