JOB SUMMARY :
The Patient Care Coordinator assists the nursing and pharmacy departments in coordinating patient care services and delivery. They are responsible for coordinating all patient visits and tracking visits through the use of the Nursing Scheduler.
REQUIREMENTS : 1. Education :
High School graduate or equivalent.
College preparation preferred.
Experience :
Good communication skills, detailed oriented and organized, experience in customer service, able to manage multiple tasks.
Excellent written and oral communication skills.
Excellent computer skills, Microsoft Word, Excel, HRIS / HCM Systems & Accounting Software
Fulltime Employment :
40hrs per week. Work week is Monday thru Friday.
Schedule determined by supervisor.
JOB RESPONSIBILITIES
Assist with coordination of patient services with other members of the health care team. Responsible for contacting nursing staff and sub-contracted nursing agencies to coordinate patient visits.
Assist with taking new referral information and entering new referrals in the Specialty Pharmacy Business Operating System systems. Relay new referral information to case management and pharmacy.
Responsible for ensuring the nurse has access to the demographic, orders, PICC information prior to the patient visit.
Responsible for documenting patient visits on the patient visit calendar in the Specialty Pharmacy Business Operating System systems including carrying patients over to next infusion cycle.
Responsible for confirming visit date / time with nurse and patient and for relaying this information to the appropriate clinical staff.
Responsible for documenting all communication regarding the patient including all visit reports in the clinical progress notes section in Specialty Pharmacy Business Operating system.
Responsible for assisting patients with non-clinical matters and forwarding clinical matters to Patient Care Manager.
Responsible for communicating with field staff daily regarding patient issues and schedule changes.
Responsible for updating daily schedules and informing the RN of any changes to the schedule.
Assist with developing the patient’s initial supply list based on the patient’s individualized care needs.
Assist with maintaining accurate and complete information in Specialty Pharmacy Business Operating system for assigned group of patients including, but not limited to : demographic information, nursing care provider, contact persons.
Assist with review of documentation to assure thorough information.
Assist with verifying that current authorization is present for release of medication and nursing services.
Responsible for sending the Plan of Treatment to the physician for signature and tracking receipt of signed POT.
Responsible for participating in QA and any other special projects as requested.
Responsible for answering phones and triaging phone calls.
Any other responsibility as designated by the employee’s supervisor.
