Duties:
- Provide comprehensive primary care services to patients in skilled nursing facilities with developmental disabilities, special needs, memory care, and dementia care.
- Perform physical examinations, diagnose and treat common acute illnesses and injuries, and manage chronic conditions.
- Order and interpret diagnostic tests, such as laboratory tests and imaging studies.
- Prescribe medications and other treatments as appropriate.
- Educate patients on preventive care measures and promote healthy lifestyle choices.
- Collaborate with other healthcare professionals to coordinate patient care and ensure continuity of care.
- Document patient encounters accurately and timely in electronic health records (EHR) system.
- Participate in quality improvement initiatives to enhance patient outcomes.
Skills:
- Excellent clinical assessment and diagnostic skills.
- Strong knowledge of ophthalmology, sterile processing, cardiac catheterization, infant care, specimen collection processing, health coaching, infection control, family planning, and other relevant areas.
- Proficiency in using electronic medical records systems (Cerner experience preferred).
- Effective communication skills to establish rapport with patients and effectively communicate complex medical information.
- Ability to work independently as well as collaboratively within a multidisciplinary healthcare team.
- Strong organizational skills to manage a diverse caseload and prioritize tasks effectively.
