POSITION: PROGRAM COORDINATOR

CLASSIFICATION CODE: EXEMPT FULL TIME

DEPARTMENT: CHILD, ADOLESCENT AND FAMILY SERVICES (CAFS)

REPORTS TO: CAFS PROGRAM MANAGER AND CLINICAL SUPERVISOR

 

STATEMENT OF JOB: This Position provides supportive mental health services to children and adolescents enrolled or eligible to be enrolled with Apache Behavioral Health Services Inc. (ABHS). The Program Coordinator provides referral to appropriate ABHS services and acts in collaboration with assigned clinical team. This position coordinates appropriate services between the client, client support system, community, the courts/ Tribal Courts and other healthcare and social service professionals. The Program Coordinator is responsible for provision of transportation services for clients and families, assisting with participation in medically necessary services, ensuring referral are assigned to proper channels. This position will also act as an advocate for the client with other service providers as appropriate. The lead position is responsible for supporting ABHS staff & community development, cultural training, review and development of program needs, and backup the Child Adolescent and Family Manager and Clinical Supervisor. The Program Coordinator will provide supervisory support when the Program Manager is not available. The Program Coordinator is also responsible for creation, coordination, facilitation, and management of department Events/Marketing.

 

DUTIES AND RESPONSIBILITIES:

  1. Manages a caseload of clients/potential clients who would benefit from behavioral health services. Screening and assessment planning, linkage, and monitoring of potential and current clients for Child Adolescent Family Services and make recommendations of continued treatment and services for the client and families.
  2. Attending meetings with community stakeholders to obtain and facilitate new referrals, report on current cases. Coordinate staff case load assignment with Child Adolescent and Family Services Program Manager and Clinical supervisor.
  3. Provides support services assistance to ABHS clinical staff, including, but not limited to, transportation, community visits, appointment setting/coordination, outreach services for clients having difficulty accessing services, and/or for reengagement in treatment services.
  4. Assist staff in helping clients in accessing needed services with service agencies in the areas of: transportation, financial assistance, housing, state/federal entitlement programs, medicine, schools, churches, and other agencies to assure continuity of care. Develop and assist staff in implementation of new Child, Adolescent and Family Services programming development and oversight of peer support.
  5. Facilitates and creates specialized groups for the CAFS program and oversees their effectiveness and use of empirical-base programs by using evidence- based curriculum or measures to monitor the effectiveness of the groups.
  6. Facilitates counseling, skill training, and education programs in coordination with the CAFS Program Manager and CAFS Clinical Supervisor. Clients are linked to relevant and effective services, all service efforts are monitored, connected and in synchrony; and pertinent information gather during assessment monitoring is provided to treatment team.
  7. Documents contact with clients to assess needs, maintain required paperwork, and engage family members. Complete assessment updates and treatment plan updates in accordance with Arizona Health Care Cost Containment System (AHCCCS) and ABHS Standards.
  8. Ability to accurately perform assessments and diagnoses with people presenting with high comorbidity of substance abuse and trauma-related manifestations under clinical supervision.
  9. Provides services at the clinic, in the community, children’s shelters, residential facilities, hospital settings and in client’s homes to ensure client obtains all benefits and services necessary. Coordinates

Oversee, conducts, schedules and trains case managers and liaisons on CFT meeting with court ordered clients for the purpose of, continuity of care, use of case management relationships, support, and structure in response to client need, flexibility of intervention strategies and facilitating client strength. Participates in interdepartmental clinical team meetings to assist and encourage consistent and timely provision of services.
  11. Provides prevention education services including presentations, workshops, and other forms public education on a variety of mental health topics, including, but not limited to, substance abuse treatment education and life skills instruction.
  12. 12.. Verify and print Medifax confirmation information for intake process as needed.
  13. Complete data entry in electronic health records all intake information for new clients.
  14. Collaborates with court/probation, tribal/community agencies, families, and other natural support systems to ensure client’s needs are met. Develops a strong working relationship with community partners. Conducts outreach, facilitates access to healthcare services and provides counseling/skills training/education/support in accordance with the Arizona Health Care Cost Containment System (AHCCCS) guidelines for mental health.
  15. Ability to demonstrate a high degree of flexibility and competence in adjusting to clientele with varying degrees of traumatic experiences and complex psychological presentations. As well as work independently to provide care to the client and families.
  16. Ability to address a variety of diverse spiritual, emotional, and social needs in multiple environments of an often-high-risk population, using a trauma-informed care approach while maintaining a high degree of professionalism and cultural awareness.
  17. Participates in interdepartmental clinical team meetings to assist and encourage consistent and timely provision of services. Participates in weekly clinical staffing with other members of the clinical team to review client progress and service requirements.
  18. Participation in all required ABHS training.
  19. Abide by all applicable policies, procedures, and program standards of ABHS, White Mountain Apache Tribe, Indian Health Services, Arizona Health Care Cost Containment System (AHCCCS) and other pertinent regulatory authorities.
  20. Maintain ethical policies and guidelines as outlined in accepted State ethics laws, rules, policies, and principles and as outlined in discipline-specific code of ethics.
  21. Will participating in the training/oversight of the Peer Support Specialist, case managers, liaisons, and specialists.
  22. Provide backup coverage for Child, Adolescent and Family Services team staff members, including clinical supervisor, for assessments, intakes, walk-ins, groups, etc.
  23. Provide a positive role model to clients and staff through leadership, self-discipline, and genuine care for the treatment and recovery of clients. All clients and staff members must be treated with dignity and respect.
  24. Must be capable of working independently creating own work schedule, highly motivated self-starter. Reports/attends work in timely manner. Must be reliable and communicate directly with supervisor in all matters of attendance.
  25. Specifically, as Lead this position serve as Point of Contact, Primary Facilitator, and Coordinator for the following Department Program Services:
    1. Quarterly Family Fun Day Events.
      1. Managing participant enrollment, tracking, scheduling, supplies, flyers, incentives, and serve as community/stakeholder liaison and point of contact.
    2. Various Department Events
      1. Community/Stakeholder Prevention, Awareness, Intervention, Education, Training, and Services (education booths, stakeholder events/training, radio appearances, etc.)
      2. Marketing 
  26. Performs other duties as assigned by supervisor.

 

MINIMUM QUALIFICATIONS:

  1. Possession of a high school diploma or high school equivalency diploma and 18 credit hours of post-high school education in a field related to behavioral health completed no more than four years before the date the individual begins providing behavioral health services and two years of full-time behavioral health work experience: or high school diploma or high school equivalency diploma and four years of full-time behavioral health work experience. Associates/bachelor’s degree in mental health/human services or related field preferred; experience working with Native Americans preferred.
  2. Excellent written and oral communication skills; ability to speak the Apache language preferred.
  3. Demonstrate strong critical thinking, problem-solving and organizational and time management skills. 4. Strong interpersonal skills with an ability to work effectively with a wide range of people, teams, managers, supervisors.
  4. MS Office proficiency and ability to correctly operate copy machine and fax.
  5. Valid Arizona Driver’s License and reliable transportation.
  6. Ability to pass all pre-employment testing, including, but not limited to, fingerprint clearance, drug testing and background check.
