Overview:
Provides case management services, including assessment, treatment planning, linkage and referral, monitoring, and advocacy to assigned caseload with an emphasis on employment, benefits, and housing. Assures intra-agency and community continuity of care for assigned caseload. Prepares and maintains client records in accordance with all applicable regulations and standards.
ESSENTIAL/CORE JOB-SPECIFIC DUTIES AND RESPONSIBILITIES
- Develops and updates individualized Client Service Plans (CSPs) for assigned cases in accordance with Client Record Handbook. Meets requirements of 90% of CSP/SPR completed.
- Meets Service Productivity standard of at least 75% (or 6 out of 8 work hours) and completes Progress Notes for all service activities in accordance with Client Record Handbook specifications and Agency business/clinical ethics standards.
Certified Behavioral Health Case Manager (CBHCM) verified through the Florida Certification Board (FCB) in accordance with Florida Statute 394.4573(e).
OR
Bachelor’s Degree in counseling; Social Work; Psychology; Criminal Justice; Nursing; Rehabilitation; Special Education; Health Education; or a Human Services Field in which major coursework includes the study of human behavior and development;
Must obtain Certified Case Manager – obtain Provisional Status (CBHCM-P) within 3 months of hire. Once provisional status is obtained, must obtain Certified Behavioral Health Case Manager (CBHCM) within 12 months. If possess the Provisional status upon hire, must obtain the full certification of CBHCM within 12 months of employment.
Licensure or Certification:
- Must maintain current Targeted Case Manager Certification.
- Must possess current, valid driver's license and maintain Apalachee Designated Driver status.
