Essential/Core Job-specific Duties And Responsibilities
- Assists clients in gaining access to needed financial and insurance benefits, employment, medical, social, education, and other services. Provides information and referrals to assist clients’ with needs such as medical, dental, occupational, residential, social and any other identified barriers to stabilization. Works with the client and the client’s natural support system to meet the needs of clients.
- Completes the community support assessment as well as other assessment forms such as, but not limited to the Bio-Psychosocial Preliminary Assessment, Medical Questionnaire, and the Medication / Verification form. Based on the assessment, develops in conjunction with the client / significant other(s), a service plan to address the needs of the client.
- Meets Service Productivity standard of at least 75% (or 6 out of 8 work hours) for the provision of Case Management services in accordance with applicable clinical/quality criteria which are supported by appropriate clinical record documentation which meet Client Record Handbook specifications, SAL documentation requirements, and Agency business/clinical ethical standards.
- Provides coordination of care between service providers, Crisis Stabilization Unit (CSU), and other necessary services. Makes appropriate referrals to community resources to assist clients to meet their needs and goals.
- Documents all services provided in the client record in accordance with the Client Record Handbook Guidelines. Progress notes must be clearly reflected how the support coordinator’s efforts are linked to the services and the client’s progress or lack of progress on each client’s goals. Maintains at least 85% compliance with progress note requirements on Clinical Reviews conducted during rating period. Program case files must be consistent with Center standards, to ensure compliance with Agency policy, F&F rules and JCAHO standards.
- Identify high risk clients and complete suicide risk assessments for all clients during the intake and/or at anytime during treatment a client are noted to have suicide ideation, plans, or an attempt. Reviews risk assessments with the Supervisor and /or MD/ARNP upon completion. Includes suicide risk, ideations, plans, or recent attempt to determine appropriate intervention to ensure the safety of the clients. Assists the client in completing the Personal Safety Plan. Maintains at least 85% compliance with assessment requirements on Clinical Reviews conducted during rating period.
- Trained in SSI/SSDI/ Outreach, Access, and Recovery (SOAR) and assist eligible clients gaining SSI/SSDI.
- Attends and participates in staff/unit meetings as required.
