Seeking a compassionate and skilled Certified Rehabilitation Registered Nurse for the role of Regional Director CRRN for Dade / Broward locations.
As a Regional Director CRRN you will be responsible for the oversight of high-quality, patient-centered care directly within the homes of the clients. The ideal candidate will possess clinical expertise, strong communication skills, and a commitment to providing comprehensive care management.
Key Responsibilities :
Home Visits and Assessments :
Conduct facility and in-home assessments to evaluate patients' healthcare needs and develop personalized care plans.
Collaborate with patients, families, and healthcare professionals to ensure care plans are tailored to individual requirements.
Care Coordination :
Coordinate and oversee the delivery of healthcare services in collaboration with multidisciplinary healthcare professionals.
Facilitate communication and collaboration between healthcare providers to optimize patient outcomes
Patient Education :
Provide education to patients and their families regarding treatment plans, medications, and self-management strategies.
Offer support and resources to enhance patient and family understanding and engagement in the care process.
Documentation and Compliance :
Maintain accurate and timely documentation of all home visits, assessments, and interventions.
Ensure compliance with regulatory standards, organizational policies, and industry best practices.
Emergency Response :
Implement crisis intervention strategies to address urgent healthcare needs of the patient.
Qualifications :
- Current certification - Certified Rehabilitation Registered Nurse (CRRN) is required. Bachelor's degree in nursing or a related field.
- Minimum of 3 years of experience Workers Compensation, Home Care, Community Health, or a related Healthcare setting.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills, especially in a field / home setting. Knowledge of regulatory requirements and standards for home care services.
- Bilingual (Preferred not required)
