We are currently seeking experienced nurse practitioners to join our team and care for the geriatric population in Skilled Nursing and Assisted Living settings. As a valued member of our healthcare team, you will play a critical role in delivering compassionate, evidence-based care that improves patient outcomes.
Key Responsibilities
Your key responsibilities will include:
- Delivering patient-centered care in Skilled Nursing and Assisted Living settings
- Collaborating with our multidisciplinary team to develop and implement comprehensive care plans
- Providing education and support to patients and their families
Requirements and Qualifications
To succeed in this role, you must possess:
- Advanced Practice Licensure
- An active license
- A DEA number
- A Master's degree
