Social Worker Hospice - LMSW

Responsibilities

The Hospice Social Worker is responsible for promoting values of compassion, integrity, and teamwork while addressing the psychosocial needs of patients and families. Key responsibilities include:

• Explaining hospice services and obtaining necessary documentation.
• Participating in interdisciplinary team (IDT) meetings and care planning.
• Providing psycho-social support and counseling.
• Conducting assessments and facilitating long-term care placements.
• Supporting advance planning and maintaining care plans.
  
  

Qualifications include a Bachelor's degree in Social Work or related field, with one year of healthcare experience preferred. Licensure is required. The role demands strong communication skills and the ability to work in a healthcare environment.
