**Job Description:**
We are hiring a Patient Care Manager with Home Health experience.
The Home Health Patient Care Manager is responsible for the supervision and coordination of clinical services and provides and directs provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies.
**Responsibilities Include:**
- Supervise and coordinate clinical services, ensuring high-quality care to patients.
- Direct provisions of nursing care to patients in their homes as prescribed by the physician.
- Evaluate patient needs and develop individualized plans of care.
- Coordinate with other members of the healthcare team to ensure seamless care delivery.
- Participate in on-call rotation.
- Candidates must have current RN licensure in state of practice.
**Required Skills and Qualifications:**
- Bachelor's degree in Nursing required.
- Current RN licensure in state of practice.
- Minimum 2 years' experience in home healthcare or related field.
