Position Summary :
The Community Health Navigator will help recruit patients for Apicha CHC’s Jackson Heights health center, particularly outreaching to Bangladeshi and / or Filipino communities in Western Queens. This role involves working closely with staff from different departments to make sure patients are smoothly connected to care. This role will conduct outreach to Asian Pacific Islanders, immigrants, and LGBTQ communities, and promote Apicha CHC’s services. The role also includes assisting with administrative tasks.
The major duties of the position include, but are not limited to :
Outreach :
- Recruit patients from priority populations
- Increase awareness of Apicha CHC services
- Educate potential clients about access to care and health services, including adult primary care, pediatrics, dental, women’s health, HIV care, HIV preventive care such as PEP, PrEP, and mental health.
- Represent Apicha Community Health Center at various health fairs and other community events
- Work closely with other program staff and volunteer community ambassadors
- Help recruit volunteers from target populations
