Service Coordinator Role
The Service Coordinator will be responsible for ensuring the effective delivery of services to our clients. This includes developing service plans, coordinating centralized intake, and working closely with the Program Manager for Support Services.
Key Responsibilities:
- Develops service plans that incorporate goals and objectives, frequency and duration of services, HIV-specific health and nutrition topics.
- Work closely with the Program Manager for Support Services to ensure appropriate services delivery to program clients, including conducting HIV Support Services program orientation, screening/re-screening, and intakes.
- Act as point person with HIV Support Services program staff to coordinate activities, assist the Program Manager for Support Services to ensure adherence to protocols and standards of Department of Support Services.
Required Skills and Qualifications:
- BA/BS in related field with two years of qualifying experience required. An advanced degree in one of the qualifying fields may be substituted for one year of qualifying experience.
- Experience working with target populations strongly preferred: HIV/AIDS individuals who are virally unsuppressed, individuals with Serious Mental Illness, serious substance use disorders (SUDs), and/or chronic hepatitis.
- Fluency in Spanish and/or Asian languages a plus.
- Excellent organization, written, verbal, and interpersonal communication skills in English required.
