Position Summary
We're looking for a culturally sensitive, enthusiastic full-time Peer Navigator for our Jackson Heights location.
Job Responsibilities
- Provide direct services to people living with HIV/AIDS under supervision of the Program Manager for Support Services.
- Assist in carrying out tasks needed to execute support service plans, including:
- Accompanying patients to appointments when required;
- Providing coaching to patients;
- Assisting with entitlements reassessment (i.e., health insurance);
- Coordinating logistics for plan adherence reminders, transportation, and/or childcare arrangements;
- Linking clients to primary care;
- Assisting in specialty referrals;
- Field-based outreach for lost-to-follow-up clients;
- Conducting home visits;
- Tracking and documenting client program participation.
- Maintain client-related records and other required documentation according to protocols and standards of Apichas Department of Support Services.
- Work as part of the unit on quality assurance and continuous quality improvement (CQI) tasks.
- Represent the agency at various venues to promote agency services.
- Other activities as assigned.
Qualifications
- High school diploma/GED with 2 years of working experience required. Some college education with 1 year of working experience preferred.
- Strong socio-cultural identification with the target population or experience in working with the target population required.
- Fluency in Spanish and/or one Asian language a plus.
- Excellent written and verbal communication skills in English required.
- Independent and good team player.
- Good attention to details.
- Ability to communicate well with medical providers and support staff.
- Ability to work well with diverse populations.
- Ability to handle multiple tasks and stressful environments.
- Ability to work flexible hours including some evenings and weekends.
- Ability to use common office software (Word, Excel, PowerPoint). Familiarity with AIRS preferred.
